Membership correction form

You can use this form if you have any corrections for the SCA Membership database. The membership number and last name are used for identification in the database; this combination is unique. First check your number in the Membership directory and secondly fill in this form. You also can send an E-mail to info@scaweb.org (with subject: Membership correction) if you're not able to submit the form.
Note: Always mention your Membership number and Last name!
